Our model, RTI RarityTM, draws on over 150 variables across 10 domains of social risk factors to create a proprietary composite Local Social Inequity score that can be used to evaluate interventions aimed at reducing disparities.
We helped a large health plan and provider system bring their data together to better integrate behavioral health into primary care. As a neutral and objective party, we reviewed the available data, identified potential gaps and challenges, and helped them define the metrics used to measure the success of the initiative.
